How to file for claims compensation
  1. The insured person, third party and/or other related parties must report the accident and request for claims compensation
  2. The insured person, third party and/or other related parties must personally submit the required documents to our Headquarter or any one of our branches nationwide
  3. We will consider all available proof/evidence (based on the documents submitted by you and/or a further report conduced by a surveyor we send out depending on the situation) and we will then inform you on the outcome
  4. We can pay out claims compensation within a given timeframe counting from the date when all related parties have agreed upon a specific compensation sum

Documents required (in the case of injury)
For initial medical expense
  1. Medical receipt from clinic / hospital
  2. Copy of national ID / foreign ID / passport
  3. Doctor's note (in the case of loss of limb or disability)
  4. Copy of surveyor's report

For additional medical expense
  1. Same as numbers 1 - 4 for initial medical expense
  2. Proof of liability from driver of insured car

Documents required (in the case of loss of life)
For initial medical expense
  1. Copy of death certificate and other proof as confirmation of being victim from the accident
  2. Copy of surveyor's report
  3. Proof of medical expense & doctor's note (in the case of having initially received medical treatment prior to loss of life)

For additional medical expense
  1. Same as numbers 1 - 3 for initial medical expense
  2. Proof of liability from driver of insured car

How to file for claims compensation
  1. The insured person, third party and/or other related parties must report the accident and request for claims compensation
  2. The insured person, third party and/or other related parties must personally submit the required documents to our Headquarter or any one of our branches nationwide
  3. We will consider all available proof/evidence (based on the documents submitted by you and/or a further report conduced by a surveyor we send out depending on the situation) and we will then inform you on the outcome
  4. We can pay out claims compensation within a given timeframe counting from the date when all related parties have agreed upon a specific compensation sum

Documents required (if the insured vehicle is damaged)
  1. Photo and/or other form(s) of proof to the company showing damage to vehicle
  2. Price quote from the garage if you intend to manage the repairment yourself
  3. Copy of daily report of inquiry officer in case of a lawsuit
  4. Any document(s) issued at the incident by our company's surveyor (if available)
  5. Letter of attorney in the case that the owner of the insured vehicle cannot deal with the case in person; In in the case of a company, please provide a company registration letter with hand signature and company stamp, as well as a bank statement book ranging back 1 year prior to the date of the accident

Documents required (if the insured vehicle is entirely damaged i.e. total loss)
  1. Photo and/or other form(s) of proof to the company showing damage to vehicle
  2. Price quote from the garage if you intend to manage the repairment yourself
  3. Copy of daily report of inquiry officer in case of a lawsuit
  4. Any document(s) issued at the incident by our company's surveyor (if available)
  5. Letter of attorney in the case that the insured person cannot deal with the case in person; In in the case of a company, please provide a company registration letter with hand signature and company stamp, as well as a bank statement book ranging back 1 year prior to the date of the accident
  6. Original insurance policy
  7. Vehicle registration book together with document of vehicle ownership transfer
  8. In the case that the vehicle is under a lease agreement and the beneficiary has not been stated on the insurance policy, the owner of the vehicle must waive the right to receive claims compensation and instead hand over authority to the beneficiary

Documents required (if the insured vehicle is lost/stolen)
  1. Copy of daily report of inquiry officer (complaint filed in criminal case)
  2. Copy of car suspension documentation which the inquiry officer sent to The Department of Land Transport
  3. Car registration book, which has been suspended by The Department of Land Transport
  4. All vehicle keys in possession
  5. In the case that the vehicle is under a lease agreement and the beneficiary has not been stated on the insurance policy, the owner of the vehicle must waive the right to receive claims compensation and instead hand over authority to the beneficiary

  1. Photo and/or other form(s) of proof to the company showing damage to vehicle

  3. Proof of ownership of property
  4. Letter of attorney in the case that the owner of the insured property cannot deal with the case in person; In in the case of a company, please provide a company registration letter with hand signature and company stamp, as well as a bank statement book ranging back 1 year prior to the date of the accident
  5. Copy of daily report of inquiry officer
  6. Any document(s) issued at the incident by our company's surveyor (if available)

How to file for claims compensation

  3. We will consider all available proof/evidence (based on the documents submitted by you and/or a further report conduced by a surveyor we send out depending on the situation) and we will then inform you on the outcome
  4. We can pay out claims compensation within a given timeframe counting from the date when all related parties have agreed upon a specific compensation sum

Documents required
  1. Details of the accident from the insured person or any witnesses from the incident
  2. Details of all damaged property
  3. Copy of national ID & copy of house registration
  4. Confirmation letter from local police (in the case of causing the fire)
  5. Letter of attorney in the case that the insured person cannot deal with the case in person, along with a copy of the national ID & a copy of the house registrataion of the person given the power to act on behalf of the insured
  6. In the case of a company, please provide a company registration letter with hand signature and company stamp, as well as a bank statement book ranging back 1 year prior to the date of the accident (in the case of a shop/store)

  • Marine Cargo Insurance

How to file for claims compensation for international transits by sea / air
  1. When the insured goods are found damaged or lost, notify one of our company representatives so that we can survey the case
  2. Demand claims compensation from the carrier, the port, or the custodian when a cargo is lost
  3. Do not sign the receipt without guided conditions whatsoever if unsure about the condition of the goods to be received unless a letter of protest has been put forward
  4. In the case of goods shipped in a container, the carrier is responsible for providing the human resource to survey the container and its seal brand immediately. If the container is damaged, the seal brand destroyed, and/or the seal brand does not match what was initially documented down, the condition it was actually found in must be noted down in the delivery receipt, and the damaged or abnormal seal brand must be kept for further review.
  5. Submit request letter to a representative of the carrier or the custodian to carry out a survey on the goods immediately if it is found that goods are damaged or lost. Subsequenty, claims compensation should be filed as soon as possible to the carrier or the other custodians based on the actual damage occurred.
  6. Notify the carrier or the other custodians in writing within 3 days counting from the day the goods were received if the damage or loss occurred was not clearly visible/present upon receival of the goods

Note
The recipient or representative should study and understand the rules & regulations of the destination port

How to file for claims compensation for domestic transits by land
  1. Inspect the condition of the goods to see where any damage or loss has occurred. If the event that damage or loss has occurred, write down the following note on the delivery receipt "Received the goods in damaged condition" thereby listing out the details of quantity and condition of goods damaged before signing that receipt, and request that the courier signs in agreement. The courier must also be willing to sign a copy of their vehicle driver's license.
  2. In the event that the truck delivering the insured goods is in an accident resulting in the goods being damaged, you must request that the courier send over documents to you in order to advance with your case for claims compensation as follows:
        2.1  In the event that there is no third party involved (i.e. overturned car) ask for a daily report from the police station
        2.2  In the event that there is a third party involved, comply as follows:
              2.2.1  In the event that you and the third party can come to an agreement and the third party is in the wrong, ask for the following documents from the courier:
                       a.  Document accepting liability for the accident
                       b.  Copy of vehicle driver's license of the third party with signature, also stating the damage occurred
              2.2.2  In the event that you and third party cannot come to an agreement, ask for the following document from the police station:
                       a.  Daily report stating the inquiry officer's opinion on the accident

Documents required
  1. Original marine cargo policy
  2. Invoice & packing list
  3. Bill of landing or master airway bill
  4. Survey report , wharf survey note, wharf receipt, short & overlanded cargo list, damage cargo list
  5. Claim letter to carrier
  6. Claim bill

Additional documents required for a domestic transit accident
  1. Copy of daily report of surveyor, or documents showing liability of the carrier or the third party for the accident
  2. Delivery slip from carrier and/or other delivery documents (original version)
  3. Copy of vehicle registration, copy of vehicle driver's license (in hand to verify the legitimacy of the the vehicle damaged from the accident and also of the third party's)
  4. Original contract to hire carrier service, and receipt of payment to hire
  5. Copy of claim letter to carrier stating the details and value damages occurred

Additional documents required for when the insured is a company / organisation
  1. Company confirmation letter with signature of a Director and stamp
  2. Copy of national ID of claimant
  3. Letter of attorney for representative receiving claims compensation on behalf of claimant
  4. Copy of national ID of representative receiving claims compensation on behalf of claimant
  5. Receipt(s) related to sum of compensation to be claimed

Note
We may ask for other supporting materials to complete your claim. Additionally, every single Marine Cargo Insurance policy clearly states the role of the insured in filing for claims compensation (the text in red). If the insured does not comply accordingly, then he/she risks losing the right to the deserved claims compensation.

How to file for claims compensation

  3. We will consider all available proof/evidence (based on the documents submitted by you and/or a further report conduced by a surveyor we send out depending on the situation) and we will then inform you on the outcome
  4. We can pay out claims compensation within a given timeframe counting from the date when all related parties have agreed upon a specific compensation sum

Documents required (in the case of loss of life)
  1. Copy of surveyor's report on date of incident 
  2. Copy of autopsy report
  3. Copy of death certificate
  4. Copy of national ID / copy of house registration
  5. Copy of passport of insured person (for foreigners)
  6. Copy of result of death investigation (if available)
  7. Original insurance policy (only for insurance policies covering individuals and not groups)
  8. Documents of beneficiary

Documents required (in the case of loss of limb or eyesight / disability)
  1. Medical report that clearly states cause of injury and limb(s) lost
  2. Medical history of patient
  3. Photo of limb(s) lost and also of entire body
  4. Copy of national ID / copy of house registration
  5. Copy of passport of insured person (for foreigners)
  6. Copy of surveyor's report on date of incident
  7. Confirmation letter of disability
  8. Documents of beneficiary

Documents required (in the case of injury from the accident) ***The insured person must pay for medical expense upfront and submit proof of payment to us to receive the total sum back***
  1. Medical report that clearly states cause of injury
